CMS’s innovation plan outlines four key processes to foster the integration of personalized and value-driven care.
The initial phase involves delivering personalized medical details to physicians, hospitals, and various healthcare entities through diverse methods and programs. This necessitates merging multiple information systems to cater to a broad spectrum of providers and stakeholders.
Starting in February 2024, the New Innovation Center began supplying shadow data to Accountable Care Organizations (ACOs) participating in the Medicare Savings Program (MSSP) and the ACO Models Reflecting Equity, Access, and Population Health (REACH). These shadow data sets offer summarized claims information that ACOs can utilize when collaborating with adjusters. This data offers a glimpse into care quality, aiding ACOs in addressing beneficiary needs more effectively.
On July 1, 2024, participants in the Primary Care (MCP) Model will gain access to performance profiles of specific practices within their regions, offering a detailed look into personalized care. These participants will also receive clear protections related to donor care and beneficiary status through a provided sample reporting tool.
